One of the characteristics that doesn’t make it into tourist brochures, however, is the less glamorous, but nonetheless, just as important detail that Spain, and the Costa del Sol in particular, have a reliable, sophisticated healthcare system that caters to both residents and visitors.

Who Is Eligible For Public Healthcare in Spain?

The eligibility criteria for the Spanish National Healthcare System (Sistema Nacional de Salud, SNS) can vary depending on several factors such as residency status, employment, and contributions to the social security system. Here are some general guidelines:

Residents

  • Spanish Citizens: Full access to the healthcare system.
  • EU Citizens: Usually eligible for healthcare if they are residents and either employed, self-employed or have sufficient financial means.
  • Non-EU Citizens: Those with a valid residency permit and who are contributing to the social security system are generally eligible.

Workers

  • Employed: If you are working and contributing to the social security system, you are eligible.
  • Self-Employed: Also, eligible if contributing to the social security system.
  • Unemployed: May still be eligible if they have exhausted their unemployment benefits but this can vary.

Others

  • Retirees: Spanish retirees are generally covered. EU retirees may also be covered depending on bilateral agreements and certain conditions.
  • Students: EU students often have access through the European Health Insurance Card (EHIC). Non-EU students may need private insurance.
  • Dependents: Spouses and children of eligible individuals are usually also eligible.

Special Cases

  • Tourists: EU tourists can use the EHIC (European Health Insurance Card) or UK citizens the new GHIC (Global Health Insurance Card) for temporary healthcare needs. Non-EU tourists are generally not covered and should have travel insurance. Visitors using these cards should know that this doesn’t provide comprehensive healthcare, for which private insurance should be bought.
  • Convenio Especial: A pay-in scheme for people who don't meet other criteria.

It's important to note that while the Spanish healthcare system is public and generally free at the point of service, some services like prescriptions may require a co-payment.

How to Register for Public Healthcare in Spain - Residents and Non-Residents

Determine Eligibility:

  • If you're a legal resident and contribute to the Spanish social security system, you're typically eligible for public healthcare.
  • If you're employed, self-employed, or have recently become unemployed in Spain, you can access public healthcare services.

Required Documents:

  • Proof of residency (e.g., Residencia or TIE).
  • Social security number.
  • Passport or national ID card.
  • Empadronamiento (a document that proves you're registered with the local town hall).

Registration Process:

  • Visit the nearest Centro de Salud (Health Center) with the required documents.
  • Fill out the necessary application forms.
  • Once approved, you'll receive a tarjeta sanitaria individual (public health card), which you should present at medical appointments.

For Non-Residents:

  • E.U. Members:
    • Present your passport or national ID card.
    • Provide the European Health Insurance Card (EHIC) or a certificate from your home country that proves you're entitled to healthcare while in Spain temporarily.
  • Non-EU Members:
    • Present your passport and any valid visas or permits.
    • Provide a health insurance certificate or a document from your home country that states you're entitled to healthcare during your stay in Spain. Note that bilateral agreements might exist between Spain and certain countries, making the process smoother.
  • Services for Non-Residents:
    • Non-residents can access emergency medical services and may receive some subsidised treatments.
    • It's essential to understand that as a non-resident, the scope of services might be limited compared to residents. It's advisable to have travel or health insurance to cover any additional medical expenses.

However, it's important to note that while the hotline aims to cater to as many individuals as possible, being addressed in any language other than Spanish is not guaranteed. Fortunately, the hotline does make provisions for those who are hard of hearing or have speech impediments, ensuring they receive the necessary support.

State Funded Health Centres (Centros de Salud)

These are the backbone of Marbella's public healthcare system and offer General Practitioner medical services as well as being a good port of call for minor emergencies. They are also known as ‘ambulatorios’ and are available for residents with the appropriate status.

  • Access: Each area has their own Centro de Salud, and to use it you'll need to register, at which point you’ll be assigned your GP. Always carry your passport and a Spanish or European health card when visiting.
  • Language Barrier: While some staff might be English-speaking, it's advisable to bring a Spanish speaker if you're not fluent.

Costa del Sol Hospital

Hospital Costa del SolThe Costa del Sol Hospital to the east of Marbella is not just a cornerstone of healthcare in Marbella and the surrounding areas; it is also highly respected across Spain and Europe. The hospital has 400 beds, with an €86m extension in progress which will add a further 150. Incidentally, an average of around 100 healthy babies are born every month – indeed, it is recognised as a leading centre for childbirth and childcare.

During the summer, the average number of emergencies increases from 140 to 355 a day, as the local population increases with the influx of tourists.
